By booking and attending a Savvy Mind Hypnosis & Coaching (SMHC)  treatment session, either in person or online, you, also known as client, acknowledges and agrees to the following Statements, Terms & Conditions and Privacy Policy: 

By attending this or any other event hosted by Michele Burghardt, d/b/a The Savvy Mind LLC including but not limited to personal sessions, workshops,  and live events, I agree not to hold Michele Burghardt or The Savvy Mind LLC responsible for any outcomes, and release, waive all right to file suit against Michele Burghardt and/or The Savvy Mind LLC for any actions taken based upon the information provided during this event. 

By attending this or any event client is aware of the types of therapy they will be receiving, including their purpose, methods and possible results and understand and consent in full to this. They understand that the treatment may be brought to an end at any time and that success of the treatment depends on the client’s ability to relax and desire to create change within themselves.  

Furthermore, it is understood that each event is for Educational Purposes Only. For Educational Purposes Only is defined as no information herein is in any way intended to replace medical, legal, psychological, or professional advice or services, nor is it able to replace the free will of any individual. 

Information contained herein does not in any manner guarantee the certainty of success and disclaims any responsibility whatsoever for any consequences resulting from any decisions or course of action undertaken by any individual pursuant to this information. 

Success:  Even though Michele Burghardt, and any SMHC representative does everything in their power to ensure success for clients, we cannot guarantee results. Savvy Mind Hypnosis & Coaching services however are guaranteed.

Guarantee

If after the first session, for any reason, you feel that hypnosis will not work for you, any payments previously made will be fully refunded and all future monies due will be null and void from that date on, no questions asked.

Recordings:  Any recordings or downloads from Michele Burghardt or The Savvy Mind LLC site or received from this practice should never be used while operating heavy machinery, driving, or performing tasks which require concentration. 

Online therapy:  is currently provided through Zoom technology.  

Intake Information: It is required that clients complete a SMHC intake questionnaire prior to actively participating in a session. This information will be utilized to achieve further success during each sessions. 

Session Rules: 

1. Sessions will not be held if client is found to be under the influence of illegal drugs or alcohol at the appointment time. In this situation, clients are ineligible for refunds and/or rescheduling of appointment. 

2. Session will only be recorded by therapist. 

Payment:  is due upon scheduling on the SMHC website or at the time of client appointment. If an on-going program is recommended, a working agreement will be drafted for the scope of services and payment plans will be described. If the total program investment is broken into payments, a $25 non-refundable processing fee is billed at the time of the first payment. If at any time during the payment period, payments are declined – all services will stop until account is made current. If there is an outstanding balance at that time, a $25 fee will be charged each month until the account is made current.  Cancellations, reschedules, delays, no shows If client schedules an appointment and is unable to maintain that time slot, 24 hours’ notice to SMHC is required to avoid a $25 no-show fee due at the appointment time.  

Hold Harmless:  Scheduling and/or attending a hypnosis coaching session signifies my understanding that this event is for educational purposes only and states that client completely releases Michele Burghardt and/or The Savvy Mind LLC of any responsibilities of any and all outcomes. By booking and attending a session client releases Michele Burghardt d/b/a The Savvy Mind LLC of any responsibility and client waives all rights to file suit against The Savvy Mind LLC and Michele Burghardt for client's based upon the information provided to them or others during a session, workshop, or event.
